Summary

 

On 20 February 2023, during night-time cargo operations in the Port of Belfast, a seafarer was working alone on the seaward side of BG RUBY, securing containers. The fall overboard was not witnessed and was only detected when the crew member stopped responding to VHF calls. The crew member was located floating face down, recovered by the pilot boat and transported ashore, but died before reaching hospital.

Lessons learned

 

Cargo-securing work near the ship’s side should not be carried out alone or without monitoring; suitable guardrails, non-slip surfaces and inflatable lifejackets should be provided; the VHF channel used on deck should be monitored from the bridge; work organisation should take account of restricted visibility, the end of the shift, the number of cranes in operation and fatigue; management and operational teams should share lessons learned and review the conditions that support safe outcomes.
