Summary

 

On 20 January 2022, BRUTIMAR was berthed at the fishing port of Figueira da Foz when a crew member fell between the vessel and the quay while boarding directly, without using the accommodation ladder. The fall was heard by the crew, who located the victim unconscious in the water. Two crew members entered the water and the casualty was recovered using the crane and a lifebuoy. Despite the assistance provided, the crew member died by drowning. The investigation identified inadequate lighting, insufficient handholds and personal factors as contributing factors.
 

 

Safety recommendations

SR01-2022/021, to the owner and operator of BRUTIMAR:

 

Assess ensuring that a safe accommodation ladder is provided whenever the vessel is alongside.

SR02-2022/021, to the owner and operator of BRUTIMAR:

Assess ensuring adequate lighting for crew embarkation and disembarkation at night while the vessel is alongside.

Lessons learned

Repeated boarding by stepping directly between the quay and the vessel does not make the practice safe; a stable, well-lit accommodation ladder suitable for the tidal conditions and relative movement should be provided; secure, non-slip handholds and footholds should be available; heavy footwear, poor visibility and changes in the distance between the vessel and the quay increase the consequences of a loss of footing; means for recovering an unconscious person should be available and familiar to the crew.
