Summary

On 26 January 2024, CASTRO was hauling trammel nets off Vila Praia de Âncora when a change in the load on the hauling line caused a heavy roll to starboard. Seawater shipped over the low freeboard and entered the machinery space through an open hatch. The vessel rapidly lost stability, capsized and sank. All four crew members reached the upturned hull and were rescued after almost three hours. One crew member was hospitalised with symptoms of hypothermia.

Safety recommendations

SR01-2024/055,

to DGRM:

Assess requiring fishing vessels with a length between perpendiculars of less than 12 m to have the same distress-alerting capability as vessels of 12 m or more, as provided for in paragraph (c) of Regulation 2, Chapter 8, of Decree-Law No. 111/2008 of 30 June. The final report does not record a response to this recommendation.

SR02-2024/055, to DGRM: Assess whether the exemption from fitting the Blue Box on fishing vessels undertaking voyages of less than 24 hours should remain in the applicable implementing order. The final report does not record a response to this recommendation.

SR03-2024/055,

to the Master of CASTRO:

Assess establishing deck-to-wheelhouse communication procedures and identifying the situations in which they should be used. The final report does not record a response to this recommendation

Lessons learned

Changes to fishing-gear loads should be communicated before tension is altered. Excessive heel, reduced freeboard and downflooding must be recognised as connected warning signs. Machinery-space openings should maintain weathertight integrity. Rapid capsizing may prevent a manual distress alert, making automatic alerting particularly important. Lifejackets supported flotation, reduced physical effort and helped the crew survive until rescue.
