Safety recommendations
SR 2026-247.1, to DENSA TANKERS LTD.:
Assess through onboard verification and feedback from seafarers, whether safety
learning from the DENSA DENIZ occurrence has been, or is being, effectively
implemented in comparable work near ships’ sides across the fleet.
Lessons learned
GAMA maritime safety investigation highlighted that:
• work near a ship’s side requires preventive barriers to remain effective throughout the task, including when a physical barrier must be opened or temporarily removed;
• risk identification and written procedures are important, but do not, by themselves, ensure a safe outcome;
• the organisation of work should provide an effective means of detecting when a person working in an exposed area may require assistance;
• emergency response measures can reduce the consequences of a man overboard occurrence, but their effectiveness depends on early detection;
• learning is strengthened when the experience of the vessel is discussed with seafarers, adapted to comparable work across the fleet and followed up to verify that the resulting measures are practical and effective;
• safety learning should include accidents, incidents and near misses, as well as successful recoveries and the practices that normally allow work to be completed safely.
