Summary

 

On 28 January 2019, the container ship EF AVA was en route between Canada and Iceland in severe weather conditions, with waves of approximately 6.5 metres and synchronous rolling reaching about 30 degrees to either side. The Master rose from his chair in the radio room and moved towards the steering position while holding a cup. During a pronounced roll, he lost his balance and struck his head on the chart table. Despite resuscitation attempts, he died on board.

Lessons learned

 

In severe rolling conditions, the bridge team should alter course or speed to avoid resonance and limit movements on the bridge; personnel already in a safe position at the controls should carry out any necessary actions whenever possible; continuous handrails and handholds should be provided along transit routes; carrying objects reduces the ability to maintain support; the risks associated with synchronous and parametric rolling should be addressed in procedures, bridge briefings and bridge weather information.
