Summary

On 22 April 2024, EF OLIVIA was adrift off the coast of Sicily, although its AIS status indicated that it was underway using engine power, when it was approached by HAYRIYE ANA. The overtaking situation and the actual movement of EF OLIVIA were interpreted differently on the two bridges. Neither bridge team converted the information available from radar, AIS and ECDIS into effective action in a timely manner. The vessels subsequently collided, sustaining significant material damage but causing no injuries or pollution.

 

 

Safety recommendations

No safety recommendations were issued because information concerning HAYRIYE ANA was not received and GAMA did not issue unilateral recommendations or conclusions.

Lessons learned

 

Although navigational data was available on both vessels, neither watchkeeper was able to translate it into effective collision-risk assessment and timely action. Target detection and continuous monitoring are essential to identify developing close-quarters situations. Watchkeepers should seek support from other crew members whenever operational demands, uncertainty or communication difficulties may affect the safety of navigation, not only during the handover of the navigational watch.
