Summary
Before 0610 on 6 May 2014, FÁBIO EDGAR was hauling pots approximately 1 to 1.5 nautical miles off Belinho, south of Castelo de Neiva, with two crew members aboard. A breaking wave struck and capsized the vessel, causing structural damage and throwing both crew members into the water. Both crew members were wearing automatic-inflation lifejackets. The fishing deckhand’s lifejacket inflated and provided flotation until recovery by SÃO RAFAEL. The Master’s lifejacket did not inflate because the gas cylinder was not correctly connected to the activation mechanism. The Master was recovered by ALMAS SANTAS and taken ashore, where death was declared after advanced life support.

SR 116-2014.1: Assess carrying out a thorough inspection, before departure, of the condition, operation and readiness of all safety equipment aboard local fishing vessels. No response.
Lessons learned
Before departure, automatic-inflation lifejackets should be checked to confirm correct assembly, secure connection of the gas cylinder, validity of the activation mechanism and general serviceability. Lifebuoys should be stowed for immediate use or arranged to release freely in an emergency. During fishing operations close to breaking-water zones, one crew member should maintain an effective lookout and monitor the vessel’s position and approaching seas.
