Summary
On 14 March 2022, during INDUSTRIAL COURAGE's voyage from Matarani to Valparaíso, an alleged collision occurred with the fishing vessel DON DIEGO I, which subsequently capsized and sank, leaving three crew members missing and three rescued. The bridge team on board INDUSTRIAL COURAGE reported no abnormal event. Analysis of the VDR data showed small non-AIS targets that were not acquired, radar and ECDIS audible alarms set to silent mode, no alterations of course or speed, and limited interaction between the bridge team and the vessel's navigational equipment.
Safety recommendations
SR01-2022/102, to ISM Company of INDUSTRIAL COURAGE: Assess the possible contribution to maritime safety of having the bridge team interact with one another, between the OOW and lookout, and with the equipment, so that their risk perception can be understood from VDR extracts.
Lessons learned
The presence of a lookout only contributes to safety when there is effective interaction with the officer of the watch; small non-AIS targets should be detected, acquired, tracked and assessed; radars, ECDIS and alarms operating in silent mode reduce the cues available to the bridge team; the VDR should enable an understanding of the bridge team's lookout, assessment and decision-making processes; a watch handover with no anomalies recorded does not, in itself, demonstrate that all risks were being monitored.
