Summary

 

On 29 March 2019, while INDUSTRIAL FUSION was berthed at Mundra Port, an 11-tonne lifting beam was unintentionally lifted after a sling caught on one of its fittings. The beam overturned and crushed a crew member between the load and the vessel’s structure. The crew member was pronounced dead at the scene

 

 

Safety recommendations

 No additional safety recommendations were issued following assessment of the actions implemented by the ISM Company.

Actions Taken

The ISM Company issued two safety circulars requiring crews to review safe working procedures, situational awareness, personal protective equipment, pre-operation briefings and safety barriers

 

Lessons learned

 

Deck teams should remain clear of heavy loads until lifting slings are fully released and the load is stable. Before hoisting, crews should verify that all slings are clear and maintain effective communication and situational awareness
