Summary

 

On 21 January 2020, MANISA STAR was berthed at Palamós during Storm Gloria. Several mooring lines had parted, and no shore linesmen or tugs were available. An able seafarer was sent onto the quay to replace the lines. During the operation, a wave washed the crew member into the harbour. A search and rescue operation was initiated, but the crew member was not recovered and was presumed dead.

 

Safety recommendations

 

No additional safety recommendations were issued following assessment of the corrective and preventive actions implemented by the ISM Company. 

Actions Taken

The ISM Company revised its safety management arrangements, required risk assessments and fleet-wide training, issued instructions for mooring in adverse weather, provided modern working lifejackets and checked the availability of safety harnesses.

 

Lessons learned

 

Risk should be reassessed whenever weather and operational conditions change, even when the same task has previously been completed successfully. Crew members exposed to a risk of being washed into the water should wear a lifejacket and, where appropriate, a safety harness and line.
