Summary

 

On 3 May 2024, the beam-trawl gear of MAR DOS SEGREDOS became snagged on the seabed. During recovery, the beam released under tension and struck the vessel. Fishing continued, but the impact had caused a loss of watertight integrity. Flooding was detected only when the high-bilge-level alarm sounded. The inflow exceeded the pumping capacity, causing loss of electrical power and propulsion. All three crew members abandoned into the liferaft and were rescued. Two sustained non-serious injuries.

 

Safety recommendations

 

R01-2024/176,

to the owner: Assess establishing an effective method for the crew to alert the Master promptly to any anomaly or occurrence they perceive. GAMA did not receive a response to the proposed safety recommendation.

SR02-2024/176, to DGRM: Assess whether paragraph 2 of Article 36 of Annex III to Decree-Law No. 280/2001 of 23 October should continue to apply when issuing safe manning certificates to wooden coastal beam-trawl fishing vessels. GAMA did not receive a response to the proposed safety recommendation.

SR03-2024/176,

to DGRM: Assess means of ensuring that fishing vessels cannot capsize without their exact position being known, taking account of the conclusions of the sonar and ROV operation. GAMA did not receive a response to the proposed safety recommendation

 

Lessons learned

 

An abnormal impact during fishing operations should trigger an immediate safety assessment before work continues. High-bilge-level alarms must provide sufficient time to locate and control water ingress. Distress alerts should be transmitted on VHF channel 16 or by DSC. Automatic position-alerting equipment is critical because even an extensive sonar and ROV search may not locate a small wooden wreck without an accurate sinking position.
