Summary

 

On 28 February 2024, the container ship MONTE DA GUIA was approaching the port of Lisbon when two crew members were separately engaged in removing container lashings. One seaman lost his balance on top of hatch cover No. 3 and fell approximately 3.4 metres onto the deck of bay 25. The crew provided basic life support and requested medical assistance. The victim was pronounced dead.

Work at height involving the removal of container lashings should not be carried out alone without monitoring; appropriate means of access, non-slip surfaces and physical barriers should be provided; the risk assessment should reflect the actual configuration of working platforms and fall hazards; management should maintain close engagement with operations, disseminate lessons learned and ensure the training and familiarisation
