Summary

 

On 2 December 2024, the general cargo ship ORKO was approaching the port of Figueira da Foz when it grounded during the port entry. Following the embarkation of the pilot, a list to port and water ingress into the vessel's tanks and compartments were detected. Despite attempts to dewater and stabilise the vessel, shore-based assistance was required. Subsequently, the vessel was towed to dry dock for assessment and repair of the damage. The occurrence resulted in structural damage to the hull and pollution and was classified as a serious maritime casualty.

 

Safety recommendations

SR01-2024/434, Figueira da Foz and Aveiro Port Administration: Assess providing pilotage services at Figueira da Foz with operational support equivalent to that available at Aveiro, including systems, system performance and the number of VTS operators, as in the Port of Aveiro.

SR02-2024/434, Figueira da Foz and Aveiro Port Administration: To assess the added value of equipping Pilots with Portable Pilot Units to be used as standard equipment, especially in cases of vessels not fitted with VDR, or S-VDR and when pilot operations are not supported by VTS operators.

SR03-2024/434, à APIBARRA: To consider the usefulness of informing its members of the conclusion of this report and the proposed safety recommendations.

Lessons learned

 

Maintaining navigational control depends on continuous information exchange and feedback between the vessel, the pilotage service, and shore-based systems. Port approach and pilot boarding require effective monitoring and the ability to anticipate developing situations. Pilotage services should be equipped with radar, voice recording capabilities, VTS support, and appropriate portable tools. Instructions transmitted prior to pilot boarding do not relieve the Master of the responsibility to critically assess the situation and exercise professional judgement.
