Summary
At approximately 0445 on 28 October 2014, PATRÍCIO departed Olhão with the Master and one fishing deckhand aboard to harvest bivalves using dredges south of the Lavaje Bar. At approximately 0545, before sunrise, the vessel began crossing the bar into a fresh to strong south-easterly wind and rough seas. While proceeding close-hauled into the waves, the Master allowed the vessel to fall slightly to port. A larger wave boarded over the starboard side, causing the vessel to heel to starboard. A second and larger wave then boarded and capsized the vessel. Both persons, who were wearing lifejackets, were thrown into the sea. The fishing deckhand was recovered at approximately 0630 by ALAMAR. The Master remained with the capsized vessel and was recovered at approximately 0715 to 0730 by M. FRANCISCO. Both casualties were taken to Olhão and then to hospital, where the Master was declared deceased. The post-mortem examination identified drowning as the cause of death.

Lessons learned
Small open fishing vessels should not attempt to cross a bar when the sea state exceeds the vessel’s safe operating capability. Available freeboard, stern trim and the weight and position of fishing gear should be considered before departure. When crossing breaking or steep seas, the vessel’s heading should be controlled continuously to avoid exposing a low side to boarding waves. Lifejackets significantly improve survival prospects but do not remove the risks created by severe sea conditions, cold-water immersion and delayed recovery.
