Casualty Event: Fall overboard

Summary

 

On 9 June 2025, the crew of SÃO GONÇALINHO used the vessel’s crane to free foreign fishing gear entangled around the starboard trawl door. While the net remained suspended and under tension, a crew member entered the danger area to fit a second sling. The first sling parted, releasing the net, which struck the crew member and caused a fall overboard. The casualty was recovered deceased after a five-hour search.

 

 

Safety recommendations

 

SR01-2025/197,

to the Company:

Assess making the use of lifejackets, or equivalent personal flotation devices, mandatory for all crew members carrying out work on deck. The company responded that it was assessing equipment compatible with the work and suitable for crew members most exposed to the risk.

SR02-2025/197,

to the Company:

Assess periodically communicating the measures contained in the risk assessment to the entire crew of SÃO GONÇALINHO, clearly identifying the actions required before each operation. The company responded that the measures were communicated to the crew and that a training session was planned for 3 November 2025.

 

 

Lessons learned

 

Stop and reassess non-routine lifting operations. Establish an exclusion zone around suspended loads and tensioned ropes, wires, nets and slings. Land or slacken the load before anyone approaches. The supervisor must maintain an unobstructed view of the operation. Slings should be identified, inspected and supported by service-life records. Suitable flotation equipment and safety helmets can mitigate the consequences but do not replace effective control of the hazard.
