Summary

 

On 10 July 2018, during cargo operations at the Bronka Terminal in Saint Petersburg, an able seafarer from SOLONG was working alone removing long lashing bars on the starboard side, between the vessel and the quay. The man overboard was not witnessed by the crew and was only detected when the crew member failed to report for the 12:00 watch. Following searches on board and on the quayside, his safety helmet was found near bay 11. A diving team subsequently located his body between the vessel and the quay.

No safety recommendations were issued in the available documentation the company implemented preventive measures following the occurrence.

Lessons learned

 

Lashing and unlashing operations near the ship’s side should be carried out by two persons, one performing the work and the other keeping watch. A work lifejacket or buoyancy aid should be worn. The officer of the watch should maintain continuous radio contact with the team. The disappearance of a crew member should immediately trigger a search on board, a check of the quayside, the suspension of operations and an alert to the authorities.
