Summary

On 7 July 2020, TREVOMAR was recovering its tender after completing purse-seine fishing operations. The winch speed was increased after the tender had reached the vessel’s side, overloading the recovery arrangement. The welded pulley support failed, releasing the pulley and tensioned wire towards the winch operator. The crew provided resuscitation and returned to Sines, where the casualty was declared deceased

 

Safety recommendations

SR01-2020/130: Assess fitting safety chains to restrain the pulley and recovery wire in the event of failure. Accepted.

SR02-2020/130: Assess marking the potential impact areas of tensioned wires on deck. Accepted.

SR03-2020/130: Assess establishing a procedure for simultaneous casualty care, shore notification and immediate medical evacuation. Accepted.

 

Lessons learned

Equipment carrying tensioned wires should have suitable secondary securing arrangements, and potential line-of-fire areas should be clearly marked and kept clear. Crew members should intervene immediately when an operation or working position does not comply with established safety measures. Medical emergencies require simultaneous casualty care, shore notification and early consideration of medical evacuation.
