Summary

 

On 11 November 2022, UHL FORTUNE was berthed at the port of Ennore, India, loading general cargo units using a ship's crane operated by shore workers. A large suspended wooden crate fell onto the deck and struck the chief officer and the third officer. The chief officer died at the scene and the third officer sustained injuries to his thigh. The investigation identified personnel exposure to the area beneath suspended loads

Lessons learned

 

No person should remain or pass beneath a suspended load. The area in which the load is being handled should be clearly demarcated, signposted and treated as a temporarily restricted zone. Each item should be inspected for damage, moisture, markings, lifting points and centre of gravity prior to loading. Items lacking adequate condition or documentation should be rejected. The responsibility of shore workers does not remove the vessel's duty to control the exposure of its crew to risk.
