Lessons learned
Access to the deck in adverse weather conditions should be subject to bridge authorisation, risk assessment, continuous communication and the use of a buddy system. Safety harnesses and lifelines should be used whenever there is a risk of water coming on deck. Critical ventilators and closures should be capable of being operated from a sheltered location. Guardrails should be designed to prevent the passage of a person's body. Checklists support the effectiveness of man overboard responses. Crew members with limited deck experience require specific familiarisation with the hazards associated with working in that environment.
