The purpose of maritime safety investigations conducted by the Maritime Accident Investigation Office (GAMA) is not to determine who failed, but to understand how the system produced the observed outcomes, identify opportunities for learningpromote lasting improvements in the safety of navigation, the protection of human life at seathe protection of the maritime environment. GAMA&rsquos investigations seek to explain, learnimprove, never to apportion blameliability.
